Health Coaches for Hypertension Control improves hypertension self-management through small-group educational sessions and support offered by trained health coaches. This program is eight 90-minute sessions that aim to increase knowledge about hypertension self-management, increase readiness to make lifestyle changes required for successful self-management, and promote behavior changes in nutrition, physical activity, stress management, tobacco use, and medication management.
This program is funded in part by the State of MN Department of Human Services Live Well at Home grant, and the Older Americans Act through the Central MN Council on Aging.
